In many cases, this can result when the bite record does not have sufficient vertical opening to make the device, particularly in the posterior region. The bite may have had to be opened artificially in manufacturing causing the posterior section of the occlusal plane to meet prematurely.  

It is always best to submit a bite record that enables enough vertical clearance throughout the arch to create flat occlusal planes. This is even more critical when a significant curve of spee exists, or if there is a deep bite or anterior open bite.

This can also happen as a result of a bite record that has insufficient material to stabilize the models, causing the models to rock into the poorly covered area.

Bite was taken at a protrusive position that is uncomfortable for the patient. Take a new less protrusive bite.

When pain is experienced on one side only it suggests that the bite was taken without proper alignment of the skeletal midlines. When the midline is deviated too far to the right or left, the joint is pulled farther on one side than the other resulting in pain and/or tenderness. Ascertain that skeletal midlines are aligned correctly for the patient and the alignment is a normal function, and retake the bite. *It is important to note any normal deviation for patient on the Rx form.

The Fusion device is ideal for those who wish to be more conservative with the patients’ initial starting position because it has extended advancement options with both the wing and the screw. The “0” wings are actually a manufacturing placeholder used until the device is delivered.

The zero wings are slightly smaller and not meant for long-term wear which may cause them to break. The best course of action, rather than replace a zero wing is to remove the wing and place the number 1 wings on the metal clips. If the 1mm amount of titration is too much, the screw can be titrated back 0.5 mms. Or a more conservative bite can be taken.

The bite record was not taken in a protrusive position. SomnoDent devices are designed to be worn in a protrusive bite position. We do not recommend fabricating the device to a centric or retruded bite record. Doing so causes the appliance mechanisms to be located farther forward in the mouth impinging on buccal tissues and interfering with proper lip seal. We recommend taking a bite that is 50%-70% of the patients’ maximum range or the most comfortable protrusive position.

The bite registration should be of sufficient VDO (vertical dimension of occlusion) all around the Mx and Md arch and include the terminal molars to allow for at least 3-5mm of acrylic thickness. This allows SomnoMed to establish a flat plane of occlusion on both the upper and lower arch as well as establish the minimum thickness of acrylic to ensure the strength of the device as well as any additional features that may be added. Super-erupted teeth, distal wrap, an open anterior bite, deep anterior bite, or steep curve of spee may require additional vertical in the bite record. Tip: Use a SomGauge and make sure the end of the fork covers the terminal molars.

SomnoMed appliances do not come standard with a distal wrap. This is an optional feature that must be indicated on the order. The SomnoMed manufacturing default is to cover the mesial ½ of the last available molar.

Covering the terminal molars usually requires additional vertical dimension in the posterior region, so it is imperative that the bite record reflect this additional VDO. One way to achieve the additional VDO is to make sure the bite fork of the SomGauge covers the entire occlusal surface of the last molar.

There was insufficient VDO in the bite record, there was an anterior open bite, or there was a deep bite. The VDO had to be artificially opened during the fabrication process to allow enough acrylic in the posterior of the device, or to wrap the distals as requested.

The bite record had insufficient vertical dimension, or thickness throughout anterior to posterior region.

A second device for the same case can be ordered at the discounted price for up to 6 months after the invoice date of the original device.

Return the device along with any broken pieces, along with U/L models and bite record. Be sure to include a repair form. Do not send personal items such as retainer cases, tool kits, or paperwork as they cannot be returned.

SomnoMed does not recommend the use of an ultrasonic cleaner for their devices.
